KOOS Study


A word about our knee pain data. The KOOS test.

Patients and customers frequently ask us what the KOOS test is and how we collect our data. KOOS is an acronym for Knee Osteoarthritis Outcome Score. It is a reliable and highly validated measure of knee pain and function. We have been collecting data for almost three years. We typically test just prior to use and then at 1,4 and 8 week intervals.

The data displayed on our home page is very good but it is not a randomized, double blind, placebo controlled study. In fact, our data represents incredibly positive outcome seen with uncomplicated arthritis. We excluded patients with large cartilage defects, an acutely torn unrepairable meniscus, unstable knees due to ligament injuries or large leg alignment defects. Although these patients show similar but highly variable results.

Nonetheless the data is real and these patients are extremely pleased with their results. The patients who do well, stunningly well, have mild to moderate osteoarthritis. Furthermore, we have every indication that the disease progression is slowed or stopped. Most become pain-free and go to "maintain mode" and treat 2 times per week or less.

Our success rate is lower with severe knee pathology but a high percentage of patients
do spectacularly well--patients we did not expect to do well--and continue to surprise us every day with their success. One common characteristic these patients share is a disciplined approach to their own care and a willingness to precisely follow our treatment instructions.

Successful Electrotherapy

Arthritis in the United States affects nearly 46 million Americans. The biggest challenge most individuals have is treating the pain and inflammation associated with the disease. Arthritis sufferers who experience significant pain find it hard to be productive and carry on normal lives. In addition, effective treatment options are wide and in most cases, ineffective. The good news is that new advances in technology and modifications to old technologies continue to show promising results for treating the disease. Electrotherapy is one such technology. Electrotherapy for the use of treating knee pain associated with a wide variety of knee ailments has received a lot of negative press in recent years, but our research has shown that small adjustments in electrical stimulation delivery and new methods in treatment can be very effective.

Electrotherapy is the use of electrical energy to stimulate muscles and nerves within the body to generate natural mechanisms to heal a wide variety of medical ailments, in this case, knee pain and inflammation. It is believed that this stimulation suppresses interleukin-1 (IL-1), which is the cytokine responsible for regulating immune and inflammatory responses in the brain. It is also believed that this releases endorphins, stimulates the Peri-Aquaductal grey area of the brain and acts on pain receptors to reduce and possibly eliminate knee pain for arthritis/osteoarthritis sufferers.

Patients in our study group have shown an 82% reduction in pain based on the scoring results from the KOOS outcome test. Our research has shown that adjustments to the frequency and sequence of traditional electrical stimulation have a dramatic effect on treating knee pain and inflammation. Additionally, traditional treatment methods involve icing the affected areas after treatment. We found that this is not necessary and, in fact, has a negative healing effect. It is our belief that more is not necessarily better and consistent steady treatment with electrical stimulation is a practical treatment approach.

We continue to make small tweaks in our methods, monitor our study group and, so far, it is showing great promise in helping patients treat their pain and inflammation and ultimately get back on their feet. It is our philosophy that patients should be cognizant of what works for them, watch their diets and exercise regulary.  We believe that these treatment methods offer a great effective alternative to those suffering from arthritis/osteoarthritis of the knee and with a careful strategy can live a normal active life.

A WORD ABOUT OUR COMPETITORS

AcuKnee competes in the market with 2 major knee electrotherapy devices: The Bionicare (bionicare.com) and The Orthocor (orthocormedical.com) knee treatment systems. We invite you to visit their websites and read what they have to say.  Both systems have demonstrated limited efficacy. Bionicare has published meaningful research which may be of interest to those treating osteoarthritis.
There are no published papers on the Orthocor device for the long term treatment of osteoarthritis. There are papers on PEMF (pulsed electronic magnetic field-the technology that Orthocor uses) for soft tissue swelling and pain showing positive effects, but this is very different from chronic osteoarthritis.

Both devices have serious limitations as far as being cost effective and a user friendly, where the treatment of osteoarthritis is concerned. The Bionicare, like the AcuKnee, is a TENS derived highly modified electrotherapy device that delivers sub-sensory threshold pulsed electrotherapy.  They require 1,500 hours of use for meaningful results. When therapy is stopped worsening of the osteoarthritis is inevitable.  It is very expensive at $750 to $2000 dollars.  It has limited insurance coverage and is, like the AcuKnee, considered investigational.  In general, Medicare will not pay for any electrotherapy of a joint and most insurance companies follow their lead.  It is not convenient or inexpensive. You cannot return it for a refund as where we offer a 30 day money back guarantee.

The Orthocor device is a Pulsed Electromagnetic Therapy Device (PEMF) that costs about $600 retail. It further requires a $7.50 replaceable timed cartridge for EACH individual treatment.  It has very limited insurance coverage and Medicare will not pay for their device.  It is effective for acute swelling.  It is considerably less useful for chronic smoldering osteoarthritis.  It is convenient to use, very expensive and, like the Bionicare, cannot be returned for a refund.

We believe our results, especially with early mild to moderate osteoarthritis, are better than Bionicare and Orthocor at a lower cost and greater convenience.  Many of our long term users with mild disease are treating once a week.  We continue to collect knee pain scores (KOOS) on everyone who will participate. Our results are impressive and will eventually be published.
